Personalized .pdf Medical/Pick-Up Forms will be received via email after you have registered and entered the necessary information on our registration site. *Returning parents: This form must be completed and returned each summer. Be sure to update any new info on the registration site so it can be reflected on your 2012 Medical/Pick-Up form.
Immunization History records can be obtained from your child’s pediatrician.

- Up-to-date Immunization History Records
- 2012 Medication Administration Form, signed (must include all perscription and non-prescription medications, including dosages, you allow Day Camp Sunshine to administer)
